Occurrences of incidents due to but not limited to design defects, mechanical failure, external factors and/or human errors are very common in the Industry. Following an incident particularly if fatalities or injuries have occurred, it is imperative for the owner operator to conduct a formal incident investigation to comply with the local regulations and/or corporate requirements. Incident investigations are challenging given the random and transient nature of the events. Often, little is known at the outset, and the investigation depends on the physical evidence collected along with interviews and documentation.
